Search Request

• Search request form
  
  When to use this form: To initiate a UK, international and/or cord blood unit (CBU) search for your patient.
  How to use this form: Fill in this form accompanied by your patient’s HLA report. For UK centres, please send this to your dedicated transplant centre (TC) inbox. For international establishments, please send this to SearchTeam@anthonynolan.org

Verification Sample Requests

• Verification typing request form
  
  When to use this form: A transplant centre laboratory or international establishment must complete this to request samples from a UK or international donor, to be tested in the transplant centre’s designated laboratory.
  How to use this form: After filling in this form, for UK centres, please send this to your dedicated transplant centre (TC) inbox. For international establishments, please send this to VTTeam@anthonynolan.org

Stem Cell Provision

• First donation request form
  
  When to use this form: To be completed when requesting an unrelated donor to donate for the first time to your patient.
  How to use this form: After completing and signing the form please send it to workups@anthonynolan.org. The form will duplicate all IDs from page one to subsequent pages once printed to PDF. Please ensure VT reports for both donor and patient are also sent and page two is completed for all concurrent requests.

• Subsequent donation request forms
  
  When to use this form: To be completed when requesting the same unrelated donor to donate again to your patient, either through PBSC, BM or DLC.
  PLEASE NOTE: If you are requesting a second transplant from a new donor please send in the first donation request forms.
  How to use this form: After completing and signing this form, please send it to workups@anthonynolan.org

• Subsequent donation request forms for an NMDP donor
  
  When to use this form: To be completed in addition to the above subsequent donation request forms when requesting an unrelated NMDP donor to donate again to your patient, either through PBSC, BM or DLC.
  How to use this form: After completing and signing this form, please send it to workups@anthonynolan.org

• Related donor sample request form
  
  How to use this form: After completing and signing this form, please send it to VTTeam@anthonynolan.org
  NB if you want to request a concurrent blood sample and donation complete the Related Donor First Donation Request Forms below.

• Related donor first donation request forms
  
  When to use this form: To be completed when requesting a familial / related donor to donate for the first time to your patient.
  How to use this form: After completing and signing this form, please send it to workups@anthonynolan.org Please ensure that VT reports for both donor and patient are also sent.

• Related donor subsequent donation request forms
  
  When to use this form: To be completed when requesting a familial / related donor to donate again to your patient, either through PBSC, BM or DLC.
  How to use this form: After completing and signing this form, please send it to workups@anthonynolan.org

• Postponement or Cancellation form
  
  When to use this form: In the event of a postponement or cancellation of transplantation.

Cord Blood Unit Requests

  When to use this form: To be completed when requesting cord blood unit(s) for shipment.
  How to use this form: After completing and signing this form, please send it to workups@anthonynolan.org Please ensure that VT reports for the patient is also sent.

  When to use this form: To be completed if testing for haemochromatosis is required.
  How to use this form: After filling in this form, please send it to ClinicalServices@anthonynolan.org

• HLA disease association testing request form
  
  When to use this form: To request a sample to be tested for HLA disease association.
  How to use this form: After filling in this form, please send it to ClinicalServices@anthonynolan.org

• Extended typing request form
  
  When to use this form: To be completed by the transplant centre or international establishment if they require Anthony Nolan to perform or request extended typing on any donor.
  How to use this form: After filling in this form, for UK centres, please send this to your dedicated transplant centre (TC) inbox. For international establishments, please send this to VTTeam@anthonynolan.org

• Histocompatibility testing request
  
  When to use this form: To request Anthony Nolan to carry out histocompatibility typing for a patient.
  How to use this form: After filling in this form, please send it to ClinicalServices@anthonynolan.org

• Cord verification HLA typing request form
  
  When to use this form: If a centre requires Anthony Nolan to carry out verification typing on a Cord Blood Unit.
